Insurances we are currently accepting for new patients:

  • Excellus BCBS (with exception of Excellus Managed Medicare, Excellus Managed Medicaid, or HMO plans, as indicated on the card)
  • Excellus BCBS Federal
  • Aetna (with exception of Aetna Pre Medicare, Managed Medicare, Aetna Managed Medicaid, or HMO plans, as indicated on the card)
  • Aetna Cornell Program for Healthy Living
  • POMCO
  • MVP (with exception of Managed Medicare or Medicaid or HMO plans, as indicated on the card)
  • CIGNA
  • CIGNA/ MVP

Note: We are not accepting new Medicare or Medicaid patients, including patients with managed Medicare, Pre Medicare or Medicaid plans offered by commercial insurance like Excellus, Aetna, MVP, Fedilis, etc.

* It is the patient’s responsibility to know their insurance benefits

* We shall inform patients of our insurance acceptance policy, but it is each patient’s responsibility to confirm that we participate with their insurance plan

New patient acceptance policy:

Before your first appointment, the reason for the initial visit needs to be clearly established. For example, an annual physical, specific problem which must be stated, and the urgency of the visit.

We may not be able to provide care for patients with certain needs or diseases based on our limitations of clinical knowledge and or resources. Patient and health care provider are to determine after initial visit if a long-term patient/physician relationship is appropriate or not.

We accept responsibility of patient care once patient has been accepted in our practice after initial office visit.

New patients who are no-show for an initial appointment, without a reasonable cause, will not be given another appointment. We will assume no responsibility of any medical care for these patients.

Please arrive 30 minutes before appointment for verification of insurance information and to complete office procedures.

  • New patients must review information about joining our practice and office policies in the "New Patients" and "Established Patients" sections of our website.
  • If we accept your insurance then you must contact our office to get further information about how to establish in our practice.
  • All new patients must complete a new patient registration and medical history via our secure patient portal before your first appointment can be offered.

New patients must bring a photo ID and current insurance cards with them at the appointment. It is our office policy to scan patient’s photo ID with insurance card and take a photograph of new patients.  These documents are linked to the patient’s chart to identify patient in the future and prevent insurance fraud.
